Years after his escape from the fiery pits, John Milton (Nicolas Cage) has tried to leave the roaring engines and hellfire behind. But the Devil’s business is never truly finished. When a series of brutal abductions sweeps across the American heartland, Milton discovers the culprits — a resurrected satanic cult operating under the blood moon, intent on sacrificing chosen children to tear open a permanent gateway between Hell and Earth.

Drawn back into the fight, Milton reunites with Piper (Amber Heard), no longer the rebellious hitchhiker he once knew, but now a battle-scarred bounty hunter with her own list of debts to settle. Together, they track the cult through desert highways, rusted ghost towns, and backwater strongholds — each mile bringing them closer to the ritual site… and deeper into enemy territory.

But this time, Hell has sent something new. The Widow — a soul-stealing assassin whose touch can rip the life out of a man mid-scream — stalks their trail, determined to drag Milton back to the pit in chains.
