In the shadowy aftermath of “Underworld: Blood Wars,” the tenuous truce between Lycans and Vampires begins to unravel. The movie opens in the ruins of a medieval castle, shrouded in mist. A team of Lycans, led by a new, enigmatic alpha named Eryon, is investigating a hidden chamber. Inside, they discover an ancient coffin with inscriptions hinting at a Vampire of unprecedented power. As they open it, the air grows heavy with an ominous presence. The screen cuts to black as a whispering voice declares, “The reign of mortals is over.”

Selene, now living in a secluded stronghold with her daughter Eve, is approached by the Vampire Council. They inform her of a rogue Vampire named Kael, awakened from centuries of slumber. Kael is revealed to be the original progenitor of the Vampire race, a being older and more powerful than any known Elder. His mission: to reclaim the throne of the Vampire world and enslave the Lycans once more.

Meanwhile, Eryon’s Lycans are facing internal strife as Kael begins to prey on their clans, turning some into mind-controlled thralls. Desperate to protect his people, Eryon reaches out to Selene for an uneasy alliance.

Selene, Eve, and Eryon form a small, covert team to track Kael’s movements. Through a series of flashbacks and lore discoveries, they learn that Kael was betrayed by the first Vampire Council and entombed to prevent his authoritarian rule. His awakening was triggered by the weakening of Vampire bloodlines and the hybridization that Selene represents.

As the team ventures deeper into Kael’s territory, Selene confronts haunting visions of her past lovers, Michael and Viktor, manipulated by Kael to weaken her resolve. Eve, now a powerful hybrid in her own right, struggles with the weight of her destiny.

Kael unleashes his full power, raising an army of shadowy creatures born from ancient Vampire alchemy. The final battle takes place in a sprawling, ruined city, with Vampires and Lycans fighting side by side against Kael’s forces.
